In this retrospective cohort (Cleveland Clinic; 53 patients with refractory RP, median treatment 24 months), abrupt discontinuation was compared with a slow taper (from weekly to fortnightly, monthly, then cessation). Recurrence occurred more often after abrupt stopping (82%) than after tapering (37%). Colchicine after stopping rilonacept delayed a flare by an average of 60.7 weeks. Most patients (80%) resumed rilonacept after a recurrence, and long-term therapy was well tolerated. A gradual taper of rilonacept thus appears associated with a lower recurrence risk than abrupt discontinuation — though larger studies are needed for a standardised tapering protocol.","created":"2026-07-03T10:32:21Z","updated":"2026-07-03T13:31:19Z","licence":"Citeer vrij, met bronvermelding en een link naar hartvaat.nl (de url van het record). In deze retrospectieve cohortstudie (Cleveland Clinic; 53 patiënten met therapieresistente RP, mediane behandelduur 24 maanden) werd abrupt stoppen vergeleken met een geleidelijke afbouw (van wekelijks naar tweewekelijks, maandelijks en dan staken). Een recidief trad vaker op na abrupt stoppen (82%) dan na afbouw (37%). Colchicine na het staken van rilonacept stelde een opvlamming gemiddeld 60,7 weken uit. De meeste patiënten (80%) hervatten rilonacept na een recidief, en langdurige behandeling werd goed verdragen. Een geleidelijke afbouw van rilonacept lijkt dus gepaard te gaan met een lager recidiefrisico dan abrupt stoppen — al zijn grotere studies nodig voor een gestandaardiseerd afbouwprotocol.","abstract_original":"<sec><st>Background/objectives</st>\n<p>Rilonacept (RI), an interleukin (IL)-1&alpha;/&beta; cytokine trap, is a biological treatment to prevent recurrence in recurrent pericarditis (RP). Discontinuation of RI in patients after 18 months with RP has been associated with a higher rate of RP. We aimed to propose a preliminary strategy for stopping Rl in select patients and evaluate their long-term outcomes.</p>\n</sec>\n<sec><st>Methods</st>\n<p>We conducted a retrospective cohort study at the pericardial disease centre at Cleveland Clinic, Ohio. Adults included had RP refractory to first-line therapies, receiving Rl for at least 12 months (median duration 24 months) with improvement in symptoms and late gadolinium enhancement on cardiac MRI. The primary outcome was recurrence of RP post abrupt Rl discontinuation versus slow taper (gradual decrease in Rl frequency from weekly, to bimonthly, to monthly and then cessation). We also evaluated the effects of colchicine prophylaxis at the end of "}
