Hierarchische Bayesiaanse netwerkmeta-analyse (PROSPERO CRD42024541215) van RCT's tussen 2005 en juni 2025 vergelijkt drie startstrategieën bij niet-acute ischemische hartziekte: optimale medische therapie, PCI bovenop OMT en CABG bovenop OMT. Het primaire eindpunt was lange-termijn all-cause mortaliteit. De analyse weegt de sterkte van het bewijs voor de comparatieve effectiviteit van revascularisatie — een onderwerp waarop de huidige ESC- en ACC/AHA-richtlijnen op dit moment uiteenlopen. A hierarchical Bayesian network meta-analysis (PROSPERO CRD42024541215) of RCTs published 2005-June 2025 compared three initial strategies for non-acute myocardial ischaemic syndromes: optimal medical therapy alone, PCI added to OMT and CABG added to OMT. The primary outcome was long-term all-cause mortality. The analysis quantifies the strength of evidence on the comparative effectiveness of revascularisation — an area where current ESC and ACC/AHA guidelines diverge — and provides a quantitative framework for decisions in stable coronary disease.

<sec><st>Background</st>
<p>Contemporary guidelines by the European Society for Cardiology and American College of Cardiology/American Heart Association for the treatment of non-acute myocardial ischaemic syndromes dispute the value of revascularisation and differ in their recommendation to perform revascularisation. A Bayesian network meta-analysis was performed, evaluating the strength of evidence for the comparative incremental effectiveness of coronary artery bypass grafting (CABG) versus percutaneous coronary intervention (PCI) over medical therapy on long-term outcomes.</p>
