Driven by rising pediatric ADHF presentations with higher morbidity/mortality and resource use than adults. Covers triage, risk stratification, diagnostic workup, initial stabilization, inotrope and diuretic choice, mechanical support thresholds, transition to chronic therapy, discharge planning, and post-discharge psychosocial follow-up. Knowledge gaps explicitly named. Aanleiding: een stijgend aantal kinderen met aangeboren en verworven hartziekte presenteert met ADHF in de Verenigde Staten, met hogere morbiditeit en mortaliteit en aanzienlijk hogere zorgconsumptie dan bij volwassenen. Het statement bestrijkt: triage en risicostratificatie op de SEH; diagnostische workup; initiële stabilisatie; keuze van inotropie en diuretica; drempels voor mechanische ondersteuning; transitie naar chronische behandeling; ontslagplanning; en psychosociale follow-up na ontslag. Het is geschreven over de raakvlakken van spoedeisende geneeskunde, cardiologie, chirurgie, intensive care en psychologie. Klinische relevantie: vult de Nederlandse multidisciplinaire pediatrisch-cardiologische zorgkaders aan; bruikbaar als referentie in kinderhartcentra (Erasmus MC, UMC Utrecht, AMC, LUMC, UMCG). Belangrijke 'knowledge gaps' worden expliciet benoemd.","abstract_original":"Nationally, there has been a rise in the number of children and adolescents with congenital and acquired heart disease presenting with acute decompensated heart failure. Compared with adults, these children have increased morbidity and mortality and use significantly more health care resources once admitted. Currently, there is little guidance on how to assess, manage, and create successful discharge plans for children presenting with acute decompensated heart failure. Given that this population represents an intersection among emergency medicine, cardiology, surgery, critical care, and psychology, a guidance document for the comprehensive management of this high-risk population is needed. This scientific statement reflects the state of current evidence and highlights important knowledge gaps in this domain. Key recommendations cover triage and risk stratification, diagnostic workup, initial stabilization, inotrope and diuretic choice, mechanical support thresholds, transition to chronic therapy, discharge planning, and post-discharge psychosocial follow-up."}
