Deze review samenvat de incidentie, timing en mechanismen van neurologische complicaties na TAVR, met nadruk op procedurele embolisatie en patiëntcomorbiditeiten. Hoewel cerebrale emboliebeschermingsapparaten en farmacologische strategieën worden onderzocht, tonen gerandomiseerde trials tot nu toe geen consistente reductie in stroke-incidentie. Een geïndividualiseerde preventieaanpak die het trombo-embolische en bloedingrisico in alle postprocedurale fasen afweegt, blijft essentieel voor de dagelijkse klinische besluitvorming.

## English: Neurologic Complications after Transcatheter Aortic Valve Replacement: An Updated Review of Incidence, Current Treatments, and Future Directions.

This review summarizes the incidence, timing, and mechanisms of neurological complications following TAVR, focusing on procedural embolization and patient comorbidities. Although cerebral embolic protection devices and pharmacological strategies are increasingly utilized, randomized trials have not yet demonstrated consistent stroke reduction. An individualized prevention strategy that carefully balances thromboembolic and bleeding risks across acute, subacute, and late post-procedural periods remains essential for daily clinical decision-making.

Transcatheter aortic valve replacement (TAVR) is the standard treatment for severe aortic stenosis, yet stroke remains a persistent complication with significant morbidity. This review summarizes the incidence, timing, mechanisms, and predictors of TAVR-related stroke, highlighting procedural embolization and patient comorbidities. We examine evidence for cerebral embolic protection devices, pharmacologic strategies, and left atrial appendage closure in mitigating risk. Despite advances in technology, randomized trials have not shown consistent stroke reduction with embolic protection. Optimal prevention requires individualized strategies balancing thromboembolic and bleeding risks across acute, subacute, and late post-TAVR periods.
