# GLP-1-agonisten verlagen sterfte bij HFpEF met obesitas, maar niet bij atriumfibrilleren

Een retrospectieve cohortstudie met propensity-matching onderzocht het effect van GLP-1-receptoragonisten op het ontstaan van atriumfibrilleren (AF) en mortaliteit bij patiënten met HFpEF, obesitas en zonder diabetes. De behandeling met GLP-1-agonisten leidde niet tot een significante daling van het AF-risico (HR 0,86; 95% BI 0,53-1,39), maar werd wel geassocieerd met een lager sterftecijfer (HR 0,42; 95% BI 0,23-0,75). Voor de dagelijkse praktijk betekent dit dat GLP-1-agonisten momenteel niet ingezet kunnen worden als preventieve maatregel tegen AF bij deze patiëntengroep, hoewel de gesignaleerde mortaliteitswinst aandacht verdient in verder onderzoek.

## English: Glucagon-Like Peptide-1 Agonists and the Incidence of Atrial Fibrillation in Obese Patients With Heart Failure With Preserved Ejection Fraction and No Diabetes.

A propensity-matched cohort study evaluated the impact of GLP-1 receptor agonists on atrial fibrillation (AF) incidence and mortality in obese patients with HFpEF and no diabetes. GLP-1 RA therapy did not significantly reduce AF risk (HR 0.86; 95% CI 0.53-1.39) but was associated with a lower mortality rate (HR 0.42; 95% CI 0.23-0.75). These real-world findings suggest that GLP-1 agonists should not be prescribed specifically for AF prevention in this population, though the observed mortality benefit warrants further prospective investigation.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) reduce cardiovascular risk in obesity, but their effect on atrial fibrillation (AF) in heart failure with preserved ejection fraction (HFpEF) remains unclear. METHODS: We analyzed TriNetX data comparing obese, non-diabetic HFpEF patients treated with GLP-1 RAs (n = 478) with untreated patients (n = 6887). Propensity matching yielded 470 patients per cohort. RESULTS: GLP-1 RA therapy was not associated with reduced AF incidence (HR 0.86; 95% CI, 0.53-1.39; p = 0.543) but was associated with lower mortality (HR 0.42; 95% CI, 0.23-0.75; p = 0.003). CONCLUSIONS: GLP-1 RAs may not reduce AF incidence in obese, HFpEF patients.
