Hartfalen en embolische gebeurtenissen komen vaak samen voor en delen pathofysiologische mechanismen zoals stasis, endoteliële dysfunctie en ontsteking. Dit overzicht beschrijft de diagnostische uitdagingen en benadrukt het belang van beeldvorming en de recente richtlijnadviezen voor anticoagulatie, met speciale aandacht voor de rol van atriale cardiomyopathie en inflammatoire processen. Voor de klinische praktijk betekent dit een scherpere afweging tussen bloeding- en trombosegevaar bij patiënten met een verminderde ventriculaire functie.

Heart failure and embolic events frequently coexist, sharing underlying mechanisms such as blood stasis, endothelial dysfunction, and systemic inflammation. This review outlines the diagnostic challenges and emphasizes updated, guideline-directed anticoagulation strategies, particularly highlighting the emerging role of atrial cardiomyopathy and inflammatory pathways. Clinicians should integrate these insights to refine thromboembolic risk assessment and tailor antithrombotic therapy in patients with compromised ventricular function.

## Abstract (original, from the publication)

Heart failure and embolism frequently coexist and share common pathophysiologic mechanisms, including stasis, endothelial dysfunction and inflammation. Their clinical presentations can often be misleading, which makes the diagnostic challenging and mandates appropriate use of different imaging techniques. This article summarizes the pathophysiological links, diagnostic challenges and management strategies of these 2 entities. It also outlines evidence-based and guideline-directed strategies for prevention and treatment, including the role of inflammation, atrial cardiomyopathy and updated anticoagulation recommendations.
